A home birth

I had an interesting start to my day today. I got a call around 2:30 AM and was told one of my missionary patients was in labor. She asked if I could come to the home she was staying at to check her to see if she should go ahead and go to the hospital or not. I agreed and when I first saw her could tell she was probably in transition. So, I checked her and found she was completely dilated. However, I also could tell the baby was not coming out head first. Her water hadn't broken yet, but I could either feel feet or hands presenting. So, I told her she needed to go ahead and come to the hospital right away. But, when she stood up to go, her water broke, and she felt a strong urge to push. Shortly after, two little baby feet started coming out. I knew we would never make it to the hospital at that point, so I told her to lay down again and proceeded to deliver her footling breech baby boy on the floor of the dining room. I had no cord clamps, suction bulb, or any other supplies. Fortunately, the baby cried right away and mom and baby were both fine. We were all amazed at the breech presentation, as the baby had definitely been head down just 4 days previously in the clinic. I was thankful that all ended well. I would have performed a c-section had I known about the breech presentation earlier in her labor, as footling breech is a high risk presentation.
